Content Overview:

 How can we support individuals with disabilities and their family members to develop a vision for a good life and find the supports they need to achieve that vision? Nationally, only about a quarter of individuals with intellectual and developmental disabilities have access to Medicaid long-term services and supports. For the majority of people with disabilities, support from their family is critical to meaningful participation in their community and to practicing self-determination in their everyday lives.
 
The “Charting the LifeCourse” framework and principles were developed by families to support and empower people with disabilities and their families to develop a vision for the future, identify how to find or develop supports, and discover what it takes to live the lives they want to live. These principles and tools can support individuals with disabilities, families and professionals to map out desired outcomes, solve problems and brainstorm, and keep in mind a holistic view of a person’s life beyond just the quality of services they may receive. Presenters will provide an overview of the LifeCourse framework and tools, share how they have used these in their own lives as siblings of people with disabilities, and address how their states engaged in the national Supporting Families Community of Practice formed around the LifeCourse framework.
